Portuguese Club of London House League Soccer – 2010 Registration for Boys and Girls

Age: 4 to 9 years old

For children born on: 2001, 2002, 2003, 2004, 2005 and 2006 ONLY!!

Registration Start Date: Saturday, February 13th, 2010 @ 1pm
Registration End Date: Saturday, March 27th, 20010 @ 3 pm

REGISTRATION DATES:
Every Saturday – Starting on February 13th until March 27th –       1:00 PM to 3:00 PM at the Portuguese Club.
NOTE: Registrations accepted after June 5th, will no longer qualify for the team picture and individual pictures must be arranged by the parents and the photographer.

AGES:

Children born on 2001, 2002, 2003, 2004, 2005, 2006.

Note: there will be no exceptions to the age limits.

Cost: $100.00 per player (includes registration fee, jersey, shorts, socks and sling bag with soccer ball), paid by cash or cheque (payable to Portuguese Club of London Soccer)

Families with more than 1 child to register:

• 1st child - $100.00
• 2nd child - $90.00
• 3rd child - $80.00

UNIFORM DISTRIBUTION & MEET THE COACH DATE & TIMES:

Saturday May 8th:

• 09:00 am to 10:30 am – children born in 2005 and 2006
• 10:30 am to 12:00 pm - children born in 2003 and 2004
• 12:00 pm to 01:30 pm - children born in 2001 and 2002

PICTURE PLANNED DATE:

Saturday June 12th, 2010.

SEASON PLANNED START DATE:

Saturday May 15th, 2010.

SEASON PLANNED END DATE:

Saturday August 21st, 2010.
